NN1731-1804 Factor VIIa Analogue Study
Invitation to Participate
We would like to invite you to take part in a research study. Before you decide you need to understand why the research is being done and what it would involve for you. Please take time to read the following information carefully. Talk to others about the study if you wish, such as friends, family, your GP.
Part 1 tells you the purpose of this study and what will happen to you if you take part.
Part 2 gives you more detailed information about the conduct of the study.

What is the purpose of this study?
The purpose of this trial is to determine the safety and efficacy (how well a condition is controlled) of an experimental drug called “activated recombinant factor VII analogue (NN1731)” for the treatment of acute joint bleeds in patients with congenital haemophilia A or B with inhibitors. Current treatment takes 2-3 injections of NovoSeven to stop a bleeding episode – this study hopes to show which dose of NN1731 is able to stop a joint bleed with a single injection.
Why have I been invited?
You are invited to take part in this study because you have haemophilia A or B with inhibitors to factor VIII or IX.
It is planned to include approximately 75 subjects in total with about 3 or 4 subjects from the UK. For you to be included in this study you will need to have experienced either of the following: at least 2 joint bleeds requiring drug treatment within the previous 6 months or at least 4 joint bleeds requiring drug treatment within the previous 12 months. These bleeds must have occurred in the ankles, knees or elbows for you to qualify for this study.
What is the drug that is being tested?
Activated Factor VII (FVIIa) is a protein normally present in the blood and has an important role in forming blood clots when a bleeding episode occurs. FVIIa can be produced in a laboratory and used as a drug when given in high dosages. As a drug recombinant FVIIa (NovoSeven®) has been on the market since 1996 and is used primarily to stop bleeding, or prevent bleeding during surgery, in haemophilia patients with inhibitors.
A new variant of recombinant FVIIa – called activated recombinant factor VII analogue (NN1731) - will be tested in this study. NN1731 is very similar to rFVIIa, but the structure of the protein has been changed slightly. Laboratory experiments suggest that the changes produce a compound that can promote a faster and more definitive cessation of bleeding.

আমরা আফনারে দাওত দিয়ার একটা গবেষনাত অংশ নেওয়ার লাগি। অংশ নেওয়ার আগে আফনারে বুঝতে অইবো গবেষনাটা কিতার লাগি করা ওর এবং আফনার কতোটুকু ভূমিকা থাকবো। সময় নিয়া নীচর তথ্য পড়বা ভালা করি। ইচ্ছা করলে ই বিষয়ে আফনে বন্ধু-বান্ধব, আত্নীয়-স্বজন আর আফনার জিপি-র লগেও মাততা পারইন ।
পয়লা অংশ আপনারে ই গবেষনার উদ্দেশ্য আর অংশ নিলে আফনার কিতা হইব তা বুঝাইবো।
দুই নম্বর অংশ আফনারে গবেষনাটা কিলান করা অইবো তা বিস্তারিত জানাইবো।
কুন্তা বুজা না গেলে বা আফনে আরো বেশি জানতে চাইলে আমরারে জিকাইবা। সময় নিবা অংশ নেওয়ার বেফারে মত ঠিক করার লাগি।
ই গবেষনার উদ্দেশ্য কিতা :
ই গবেষনা বা ট্রায়ালর উদ্দেশ্য অইলো ইনহিবিটর সহ কনজেনিটাল হেমোফিলিয়া ‘এ’ বা ‘বি’ –ত আক্রান্ত রুগির শরীরো তীব্র জয়েন্ট রক্তপাতর (acute joint bleed) চিকিৎসার লাগি “এক্টিভেটেড রিকমবিনেন্ট ফ্যাক্টর ৭ এনালগ [NN1731]”-নামর একটা পরীক্ষামুলক ঔষধর কার্য্যকারীতা এবং নিরাপত্তা পরীক্ষা করা (কত ভালা ভাবে একটা অবস্থা নিয়ন্ত্রিত অয়)। বর্তমান চিকিৎসা পদ্ধতিত এক দফা রক্তপাত বন্ধ করতে নভোসেভেন-অর ২-৩টা ইঞ্জেকশন লাগে – ই গবেষনার আশা অইলো NN1731 কুন মাত্রায় প্রয়োগ করলে জয়েন্টর রক্তপাত একটা মাত্র ইঞ্জেকশনউ থামবো তা দেখা।
আমারে কেনে দাওত দেওয়া হইল :
ই গবেষনাত আফনারে অংশগ্রহণ করার লাগি দাওত দেওয়া হইছে কারন আফনে ৮ বা ৯ ফ্যাক্টরর ইনহিবিটর-সহ হেমোফিলিয়া ‘এ’ অথবা ‘বি’ অসুখে ভুগরা। পরিকল্পনা আছে মুটামুটি ৭৫ জনরে ই গবেষনাত অন্তর্ভূক্ত করা, যার মাঝে বিলাত থাকি থাকবা ৩/৪ জন। ইকানো ঢুকার লাগি আফনার নিচর যে কোন একটা অভিজ্ঞতা থাকা লাগবোঃ কমপক্ষে ২টা জয়েন্ট-রক্তপাত যার লাগি গত ৬ মাসর মাঝে ঔষধ নিতে অইছে অথবা কমপক্ষে ৪টা জয়েন্ট-রক্তপাত যার লাগি গত ১২ মাসর মাঝে ঔষধ নিতে অইছে। গবেষনাত অংশ নেওয়ার লাগি যুগ্য অইতে অইলে আফনার গুড়ালি, আটু, বা কনুইত ইলান রক্তপাত অওয়া লাগবো।
যে ঔষধ পরীক্ষা করা অর ইটা কিতা ?
এক্টিভেটেড ফ্যাক্টর ৭ (FVIIa) একটা প্রোটিন যা স্বাভাবিকভাবেই রক্তে থাকে আর যখন রক্তপাতর ঘটনা ঘটে তখন রক্ত জমাট (clot) বাঁধায় গুরুত্বপূর্ন ভূমিকা রাখে। FVIIa গবেষনাগারো তৈরী করা যায় এবং রড় মাত্রায় দিলে ঔষধ হিসাবে ব্যবহার করা যায়। ১৯৯৬ থাকি ঔষধ হিসাবে রিকমবিনেন্ট FVIIa (নভোসেভেন®) বাজারো আছে আর প্রধানত ইনহিবিটরযুক্ত হেমোফিলিয়া রুগীর বেলাত রক্তপাত থামানির লাগি বা অপারেশনের সময় রক্তপাত না অওয়ার লাগি ব্যবহার করা অয়। ই গবেষনাত এক্টিভেটেড রিকমবিনেন্ট ফ্যাক্টর ৭ এনালগ (NN1731) নামর রিকমবিনেন্ট FVIIa-র একটা নতুন ভিন্নরূপ পরীক্ষা করা অইবো। NN1731-অর লগে FVIIa-অর খুব মিল আছে, কিন্তু পয়লাটার প্রোটিনর গঠন কিছুটা বদলানি অইছে। ই বদলানি একটা যৌগর সৃষ্টি করে যা আরো তাড়াতাড়ি আর বেশি নিশ্চিতভাবে রক্তপাত থামায়।

I am a native Bengali & Sylheti speaking freelance translator and language services provider, based in Bangladesh. I have been doing translation work for 20+ years on a free lance basis. I have translated various English language TV programmes (English to Bengali) for the most popular Bangladeshi TV channel Ekushey Television Ltd., as well as for other channels. I also translated technical books, manuals, information leaflets, etc for both governmental and non-governmental and foreign organisations . I have also translated various literary pieces of famous authors and published articles in both Bengali and English language national dailies. I am also familiar with information technology and Internet related jargon.
My experience in various kinds of translation range from the extremely audience-friendly translation of the documentary TV series “Toyota World of Wild Life” aired on Ekushey Television of Bangladesh, or the manual of neo-natal child care for “Save the Children”, to the quite technical translation of the “Guidelines For Earthquake Resistant Non-Engineered Construction. The intended level of lucidity for the translated version of the “Toyota World of Wild Life” was that it should be comprehensible and enjoyable even to students of or, people who received schooling upto, Class- VII / VIII, while the Save the Children’s manual was intended to be accessible to mid-level health workers around the country dealing especially with asphyxiate children. On the other hand, the “Guidelines For Earthquake Resistant Non-Engineered Construction” was intended for engineers and people in decision making and senior positions with engineering/relevant science background. In 2006, another book (bilingual) on Earthquake Engineering, the “Earthquake Resistant Design Manual”, came out which too I have translated. It's jointly published by Bangladesh Earthquake Society and APPL (Academic Press and Publishers Limited), Dhaka. The translated versions of these earthquake books are of pioneering nature in terms of translation, since there is no precedent work in Bengali language of similar nature. Translation of these very highly technical books posed a great challenge, not just because they were very technical – but also because there were no books on the subject in Bengali, no glossaries and very few relevant terminologies. The translation required a strong and appropriate balance/combination between linguistic skills and capacity to understand new technical matters/concepts, ability to translate, transcreate and discriminate conceptually, perceptively and discerningly, and retain or innovate critically - if and when necessary. The successful translation, completion and eventual publication of these books are a testament to my ability and show that I met all the challenges and requirements mentioned above.
I am also familiar with principles of good practice [ISPOR] for the translation and cultural adaptation process for Patient-Reported Outcomes Measures and the role of a local project coordinator in this regard.
Therefore, in light of the brief description above, I believe I have a relatively wide range of capacity to translate and if needs be, culturally and/or intellectually adapt materials on various subjects for a diverse
audience : both targeted and general.
I am committed to quality and punctuality. I fully appreciate and respect the concerns of agencies and other outsourcers (and the risks they take) regarding the quality and timely delivery of the output/product -- when they assign a job to a new translator/vendor. Especially when it’s done over the net and the vendor is a freelancer or based overseas. Therefore, I offer a firm commitment and assurance to QUALITY, instead of a cheap shoddy bargain product which may cause irreparable damage both to the end-customer and to the goodwill or reputation of the agency/outsourcer involved. This is why my motto is : “Faithfully and Effectively, Perceptively and Discerningly.”
I have always been deeply interested in Translating. But apart from translation, I am interested in a wide variety of other subjects too. I have a long-standing love for literature -- both in Bangla and in other languages. My interest in this field includes politics, history, religion, social issues, fiction and poetry. I am also in tune with current affairs.
Apart from my individual expertise : I am also in a position to receive collaboration, if needs be, from a diverse network of people with experience and expertise in translation activities, ranging from professional translators to free-lancers and part-timers, generalists to experts, working in diverse professions.
I am fortunate to be able to draw on the expertise and services of these people if necessary. This will also enable me to take on a large volume of work necessitating a collective effort or work requiring specialist/expert knowledge necessitating the services of pertinent professionals.
Therefore I believe, with my skill, experience and the optional human resources available to me as described above, I will be able to carry out any assignments given to me -- maintaining Quality and Punctuality.
Finally, I am also able to undertake any kind of Voice-Over, Voice Recording or Transcription projects in Bengali and Sylheti. I am also interested in teaching the Bengali language via Internet and email.
