Maggie was resting her elbow on the table, leaning her head on her hand and looking at Philip with half-penitent dependent affection, as she said this; while he was returning her gaze with an expression that, to her consciousness, gradually became less vague,–became charged with a specific recollection. Had his mind flown back to something that she now remembered,–something about a lover of Lucy's? It was a thought that made her shudder; it gave new definiteness to her present position, and to the tendency of what had happened the evening before. She moved her arm from the table, urged to change her position by that positive physical oppression at the heart that sometimes accompanies a sudden mental pang.

one of the meanings of "positive" is "clear" and in Millennium Dictionary is is translated as "آشکار/ مطلق...."Considering the context, I think it is what meant here.

  -> thank you; However, I think "پریشانی" is a good option too; especially that it is a literary text. Literary discourse is typical for its خرق عادت!if I were to translate this text, I would use even آشفتگی آشکار because of the alliteration! :)

Explanation:
فشار شدیدی در قلبش که گاهی با سوزشی دردناک در روح همراه است، باعث شد حالت خود را تغییر دهد
قلبش تیر کشید و با ضربه ای ناگهانی که حس کرد به روحش وارد شد، حالت خود را تغییر داد

As this is a literary text and not an informative one, you need not to follow the words exactly as they are and translate them literally. You can even change the structure. But if it is only the meaning of this phrase that you're looking for, that's another issue.

Oppression at the heart means pressure felt at the heart. It is a tensive pain felt at the heart. That's why I think it is equal to تیر کشیدن in farsi.
Positive here is used for emphasis as in the journey was a positive nightmare. It also means severe.
