Only now I thought of something else here. Are you sure this evaluation refers to the patient? Could this be an evaluation of the hospital\'s clinical services/facilities? Because the expression could imply both things: (1) A clinical evaluation of the hospital or (2) a clinical evaluation of a patient done by the hospital.
I hid my answer because I\'m interpreting this expression differently now. The translation depends on the issue above.

\"Avaliação clínica\" foi utilizado desta maneira em outra parte do documento. \"Clinical evaluation\" é o que as companhias de seguros exigem, a fim de conceder uma autorização prévia.

@Michael, I believe it is a clinical assessment, since clinical evaluation seems to refer to other settings, like the evaluation of medical devices prior to approval.
