This is the salutation of a medical letter seemingly from one medical practice to another - the name after 'Fam.' is not included and there is no address above for reference. The letter includes detailed anamnesis, diagnoses, findings, etc., and thanks the addressee for the referral.

I would think Fam. is for Familie, though the specificity of the details included seem to indicate they are addressing another medical professional. Has anyone encountered this before?

Explanation:
I have seen this as a typo for Fma. (Firma) – company.
However, Familienpraxis – family practice – might also fit the bill.
That said, perhaps this is just an example of hasty work on the part of the issuing practice?
