I've watched some videos of the test and looked at refs but still can't figure out what this is supposed to mean.


Tests for diagnosing epicondylitis. Describing the Mill's test/manoeuvre:

"El paciente está sentado, con el hombro separado hasta la horizontal, en rotación interna. Normalmente suele haber un aumento del dolor al extender de forma pasiva el codo, manteniendo el antebrazo en pronación y la muñeca y los dedos en máxima flexión."

shoulder in horizontal abduction

Patient seated with the affected extremity in 90° abduction and internal rotation (olecranon faced up).Therapist stands behind the patient , stabilized the patient’s wrist in full pronation and flexion, while the other hand is placed on the olecranon. The maintenance of full wrist flexion is important as failure to maintain this position could lead to a majority of the thrusting force being bome by the humeroulnar joint.

I think it means that the elbow is held at the same height as the shoulder, whether the patient is standing or sitting. If they're sitting, they can rest their arm on a table.

You can also say 'shoulder abducted to 90º' or 'shoulder in 90º abduction'.

* 'If they're sitting, they can rest their arm on a table'. I wrote that before I had watched videos of Mill's manipulation. It's obvious that in this case, patients can't rest their arm on a table!

the shoulder internally rotated in horizontal abduction / adduction

The patient is seated, with the shoulder internally rotated in horizontal abduction / adduction. Usually pain increases when passively extending the elbow, keeping the forearm pronated with the wrist and fingers fully flexed.

Supongo que hay varias formas de expresarlo. La más directa y clara me parece esta;
The patient is seated with humerus horizontal at 90 degrees and (shoulder) in internal rotation.
También, por ejemplo, arm elevated at 90 degrees of abduction.

Lo que describe el original básicamente es a un paciente sentado con un brazo "en cruz" y con el hombro correspondiente internamente rotado.
