Ophthalmology
Taken from the results of an eye test. It appears at two in the patient's history. First, in patient history:
"Cia Rx. Portador de LCVH 10h/dias (4 días sin)
AP: Sano. No alergías ni ttos. No cias."
This is then followed by the test results. I'm afraid I havent got much else!

Discussion
Rx por otro lado, y a pesar del limitadísimo contexto, diría que alude a refractiva (cirugía refractiva (previa); cia Rx; refractive surgery).
Refractive surgery. [Currently using soft lenses 10 hours per day...]
Past medical history: No past illnesses or [other] surgeries, no known allergies and no current medication.
The seeming contradiction (no past surgeries / refractive surgery) is present as patient's current clinical history is probably being written by the ophtalmologists who follow the patient after refractive surgery...
Is there any indication what LCVH might be? Some sort of lens perhaps?
Rx is usually X-ray (although it doesn't really fit in with ophthalmology), it could also be "resección" according to Cosnautas.
