Mi duda es donde habla de la periferia del segmento....

Periphery is a suitable translation of "periferia", IMO.
There are various ways of expressing "segmento VII hepático" > "liver segment VII", "segment VII of the liver" or "hepatic segment VII". Many sources also use Arabic numerals, but the Roman numerals come from the Couinaud classification.

The Couinaud classification of liver anatomy divides the liver into eight functionally indepedent segments.
Each segment has its own vascular inflow, outflow and biliary drainage.
In the centre of each segment there is a branch of the portal vein, hepatic artery and bile duct.
In the periphery of each segment there is vascular outflow through the hepatic veins.

I do work as a Nurse and know what I am talking about. When we say "en la periferia" we do not mean "on the surface" as Muriel states in her explanation. Due to the segment characteristics -and I know what I am talking about, it is not on the segment surface. The most common area in those cases the diaphragm.

disagree  Muriel Vasconcellos: Not 'the surroundings of' - that would be for a geographic neighborhood. Also, it's on the periphery, not outside it.

  -> Después de ver mi respuesta es fácil dar la tuya, cuatro horas después. "En la periferia" means "outside", not " inside the segment" as you state in the comment to my response.
