迷われる気持ちは分かりますが、わたしならこうすると思います。
カタカナでポジティブ/ネガティブでも大丈夫ですが、できるだけカタカナ語は避けたいものです。ただしこの場合肯定的、否定的という言葉はおそらく英語の訳語として存在するので、あまりこなれた日本語ではなく、いっそ元の英語のカタカナを使うのも悪くないと思います。
「良い/悪い」の方がこなれた日本語ですが、少し意味が異なるので、実際の質問をみなければ安心して使えない感じです。

This is just another suggestion in case the questionnaire targets the general public including people with minimum education and children (such as upper primary school students). If the target is university students, I think the other suggestion is perfectly fine. It's just that using easier words makes the questionnaire usable for a wider range of people.
