What are the possible meanings of this word? Can it mean the same as 'social rehabilitation'?

reintroduction into society

I would interpret this as reintroduction into society. Hits on Google suggest that it is often used to refer to criminals, etc. coming back into normal society after imprisonment.

Social rehabilitation sounds like a good option as well.

reintegration

is what it basically means IMHO.
Having become reintegrated, one can be achieve sociál rehabilitation (acceptance of peers and the like)
