1. Un documento de Informe de diagnóstico en el que se reflejen los principales hallazgos,
Recomendaciones y lecciones aprendidas de todo el proceso, a fin de que sirva para la toma de decisiones en cuanto a la factibilidad de una intervención de la Organizacion en los territorios.
2. Que las comunidades conozcan a fondo su realidad, como resultado del proceso de su participación en el estudio y la socialización de los resultados logrados en el diagnóstico, permitiendo establecer una relación de comunicación y fluidez entre la Organizacion y las comunidades lo que facilitará el trabajo a futuro.

This a type of report, not a report about a medical diagnosis, but a type of report. Therefore, shouldn't we use the adjective form "diagnostic report" rather than noun + noun form of "diagnosis report".
A "statistical report" would present the statistics (obviously) and then those statistics are analysed or summarised in a "diagnostic report".
"Diagnostic report" gives a lot more hits than "diagnosis report" (380,000 cf. 60,000) and you can see that they refer to a broad spectrum of subjects.....
