Thanks Frau Krueger and Frau Verceles. Although nowhere % sign is mentioned, based on calculation it is clear that it is 100% and 94%. I am confused with the meaning of the apostrophe. In English translated text, I should add % sign after rates then.

Maybe it’s an OCR error? I’m noticing that in 100'.00 and '94.00 a straight apostrophe is used while in 5’229.00 and 4’915.25 it’s a slanted apostrophe.

I am not aware of this particular case, but in general, it means the factor/percentage that a municipality levies on the tax the cantons/state charges. In your case: 100% means the canton/state tax rate and 94% is the individual municipal rate. They are both needed to calculate your individual tax rate. You can also see the relation when calculating back and forth the amounts stated in your text.
