What is the best translation for the phrase (broken Appointment) into English?

Broken Appointment Policy

Because of limited space, once an appointment is reserved for your child, it is important that you make every effort to keep it. We make every effort to confirm your appointment 48 hours in advance as a courtesy to ensure that your child has an opportunity to be seen in our center.

If you need to cancel or change a scheduled appointment, we ask that you notify us at least 24 hours in advance.

A broken appointment is considered to be any of the following:
1. Arriving more than 15 minutes late for your scheduled appointment.
2. Failing to cancel an appointment at least 24 hours in advance.
3. Not showing up for the scheduled appointment.
