Not 'afdeling' The abbreviation for 'afdeling' is 'afd.', not 'af'.

The number, which follows the abbreviation is likely to be the employee number or code of the person at the bank who 'pushed the buttons' to generate the statement (assuming that this was not a regularly issued statement but a statement printed at the request of the account holder). Alternatively, it could, possibly, be number of the branch at which the statement was printed - or even the number of the department at that branch.
