Le domicilié donne mandat à ENTREPRISE XY, qui l'accepte, de recevoir en son nom toute notification sans restriction de forme ou de provenance.
Article 6
Le contrat de domiciliation commerciale est conclu pour une durée de trois mois renouvelable par tacite reconduction.
Les conventions de réexpédition de courriers et options sont conclues pour une durée minimum de un mois, renouvelable également par tacite reconduction.
Article 7
Les deux parties se réservent le droit de résilier le présent contrat à tout moment en respectant un préavis de trois mois avant l'échéance du contrat, par lettre recommandée avec accusé de réception et sans que celui-ci puisse entraîner le paiement d'une indemnité. La résiliation définitive interviendra 3 mois après la réception de la demande de résiliation. Le domicilié devra fournir, à l'issue de cette période, un justificatif de transfert effectif ou de radiation effective du Registre du Commerce et des Sociétés ou du Registre des Métiers. A défaut de la fourniture du justificatif de transfert ou de radiation dans un délai de 3 mois, le contrat de domiciliation sera renouvelé par tacite reconduction par période d'un mois, facturé au tarif en vigueur, jusqu'à la fourniture de celui-ci. La résiliation est de plein droit et à effet immédiat, en cas de défaut de paiement d'une seule facture à son échéance, d'inexécution d'une des clauses du contrat par le domicilié ou de troubles occasionnés au bon fonctionnement de l’ENTREPRISE XY. Cette résiliation est notifiée par tout moyen au siège social du domicilié, par courriel à l'adresse fournie par le représentant légal du domicilié dans le présent contrat, adresse servant entre autre la notification des courriers du domicilié.
Les frais de relance écrite pour toute facture impayée sont fixés forfaitairement à 7,00 € TTC l'unité.

Tous les frais occasionnés au domiciliataire par la défaillance du domicilié, seront refacturés au domicilié majoré de la TVA.
En cas de résiliation de contrat du fait du domiciliataire ou du domicilié, les courriers, plis, magazines... non récupérés au jour de la résiliation et archivés par le domiciliataire passé le délai d'un an calendaire à compter de la date de résiliation du contrat, seront systématiquement détruits par le Domiciliataire. Charge au domiciliée de venir récupérer son courrier dans le délai au jour de la résiliation ou d'en demander la réexpédition aux conditions et tarifs en vigueur au jour de la demande.
Article 8
Une avance de garantie correspondant à trois mois de loyer de domiciliation commerciale T.T.C, est versée ce jour, sauf pour les contrats faisant l'objet d'un règlement annuel. Cette avance, non productive d'intérêts est remboursable au terme du contrat. Toutefois, en cas de résiliation pour défaut de paiement le dépôt de garantie est conservé par le domiciliataire à titre d'indemnité. Les sommes non réglées en principal et à titre annexe restant dues par le domicilié.
Article 9
Le domicilié remet à l’ENTREPRISE XY une attestation sur l'honneur précisant le lieu où est tenue et archivée la comptabilité. Cette attestation est annexée au présent contrat. Le domicilié s'engage également en cas de vérification de comptabilité, à mettre les documents nécessaires à la disposition de l'Administration à l'adresse de la domiciliation.

Article 6

Article 7
Both parties reserve the right to terminate this contract at any time, with a period of three months' notice prior to the expiry of the contract, by registered letter with an acknowledgment of receipt, which shall not result in the payment of compensation. The costs for a written notice of any unpaid invoice are fixed at a flat rate of € 7.00 including tax per unit.

Sunshine es recibido a la edad de 4 meses y medio. Pesaba 5 kg por 51 cm. Él termina una pequeña gastroenteritis, que se trata bajo Tiorfan. Contrajo gastroenteritis nosocomial por rotavirus.
El 23/01/2017, episodio de diarrea en relación con la leche anti-regurgitación. Paso a una leche 1st edad, regurgitaciónes dolorosas. Tratamiento con Inexium iniciado.
El 27/01/2017, tiene una conjuntivitis que cederá rápidamente tras el tratamiento con Azyter. Del 28/01/2017 al 03/02/2017, Sunshine está hospitalizada por bronquiolitis, una neumonía
derecha tratada, y conjuntivitis. Estuvo por tres días bajo oxígeno con nutrición parenteral
(tubo gástrico) y antibióticos (amoxilina) durante 8 días.
Desde el 2 de febrero de 2017 hasta el 13 de febrero de 2017, se reingresó por fiebre y síndrome de gripe (nutrición parenteral durante 4 días).
El 22/02/2017, Sunshine es hipertérmico, lo que requerirá la vigilancia de las enfermeras de la guadería.

Desde el 04/04/2017 al 07/04/2017, se reingresó para la rehidratación intravenosa por
infusión por tercera vez desde su recepción. Se diagnostica la intolerancia a la proteína de vaca. Él aún está en tratamiento con Inexium y tiene una leche sin proteína de leche de vaca. Una cita con el alergólogo se establece el 07/03/2017 a las 10:30.

Source text - English § 1 Warranty, product liability
10.1 The Distributor is obliged to fulfil justified warranty claims by customers. The Distributor will inform TOPICS Medical in good time about any warranty claim that results or could result in a legal dispute due to specific evidence. In addition, the provisions in § 8 Number 4 apply.
10.2 The Distributor will inform TOPICS Medical without delay about any product faults and risks of which he becomes aware when using the Contractual Products.
This agreement does not affect TOPICS Medical's product liability as stated in Number 4 below.
10.3 The Distributor will not modify Contractual Products without the prior agreement of TOPICS Medical. This also applies with regard to their features, packaging and warnings. If the Distributor infringes these obligations, he is liable to TOPICS Medical internally for product liability claims by third parties if and to the extent to which the damage was caused by the behaviour of the Distributor.
10.4 The Distributor shall maintain adequate product liability insurance with a reasonable coverage and provide TOPICS Medical with a confirmation immediately upon its request. In particular, such insurance shall cover pecuniary loss and recall costs (extended product liability). In the event that any recall measures, field corrections or corrective and preventive action (“CAPA”) is carried out by TOPICS Medical, the Distributor shall be obliged to use best efforts to assist TOPICS Medical at the Distributor’s own expense.
10.5.a) In the event of injury or loss of life, or damage to something other than the Product being caused due to a Product being defective, TOPICS Medical’s liability shall not exceed the maximum amount of EUR three (3) Mio during the term of this Contract. For all other damages, TOPICS Medical’s liability shall not exceed 100% of the purchase price of the shipment, which included the defective or delayed Products, irrespective of whether the claim is based on contract or on the statutory provisions. In the event of intent, TOPICS Medical’s liability shall not be limited.
§ 2 Contractual term and termination
11.1.a) The agreement starts on June 30th, 2015 and has been concluded for the EU: for the term of five years. Both contractual parties may terminate the agreement whilst complying with a notice period of (three) 3 months to the end of a calendar month. The agreement may not be terminated to end after less than (one) 1 year of validity.
11.2 The right to extraordinary cancellation for a justifiable cause remains unaffected. The following are considered to be some of the justifiable causes for a termination by TOPICS Medical.

Cette assurance doit notamment couvrir les pertes pécuniaires et les frais de retrait (responsabilité élargie du fabricant). Dans l’éventualité où des mesures de rappel, des services correctionnels ou des actions correctives et préventives seraient mises en oeuvre par TOPICS Medical, le distributeur serait tenu de déployer tous les efforts pour aider TOPICS Medical aux frais du distributeur.
10.5.a) En cas de blessure, de mort, ou de dommages visant autre chose que le produit en raison d’un produit défectueux, la responsabilité de TOPICS Medical ne peut dépsser le montant maximal de trois (3) millions d’euros pendant la durée du présent contrat. Pour tout autre dommage, la responsabilité de TOPICS Medical ne peut dépasser 100 % du prix d'achat de l'expédition, qui comprenait les produits défectueux ou retardés, indépendamment du fait que la réclamation soit fondée sur un contrat ou sur les dispositions législatives. En cas de manquement délibéré, la responsabilité de TOPICS Medical n'est pas limitée.
§ 2 Durée du contrat et résiliation
11.1.a) L'accord prend effet à compter du 30 juin 2015, et a été conclu pour l’UE : pour une période de cinq ans. Source text - English XENTICS can be used with or without topical corticosteroids. Topical calcineurin inhibitors may be used, but should be reserved for problem areas only, such as the face, neck, intertriginous and genital areas.
If a dose is missed, instruct the patient to administer the injection within 7 days from the missed
dose and then resume the patient's original schedule. If the missed dose is not administered within 7 days, instruct the patient to wait until the next dose on the original schedule.
Important Administration Instructions
XENTICS is intended for use under the guidance of a healthcare provider. A patient may selfinject
XENTICS after training in subcutaneous injection technique using the pre-filled syringe.
Provide proper training to patients and/or caregivers on the preparation and administration of
XENTICS prior to use according to the “Instructions for Use”.
For the initial 600 mg dose, administer each of the two XENTICS 300 mg injections at different injection sites.
Administer subcutaneous injection into the thigh or abdomen, except for the 2 inches (5 cm) around the navel. The upper arm can also be used if a caregiver administers the injection.
Rotate the injection site with each injection. DO NOT inject XENTICS into skin that is tender, damaged, bruised, or scarred.
There are two presentations for XENTICS (i.e., pre-filled syringe with or without needle shield). The XENTICS “Instructions for Use” for each presentation contains more detailed instructions on the preparation and administration of XENTICS
Hypersensitivity
Reactions, including generalized urticaria and serum sickness or serum sicknesslike reactions, were reported in less than 1% of subjects who received XENTICS in clinical trials. Two subjects experienced serum sickness or serum sickness-like reactions that were associated with high titers of antibodies to dupilumab.
Interactions with CYP450 Substrates
The formation of CYP450 enzymes can be altered by increased levels of certain cytokines (e.g., IL-1, IL-4, IL-6, IL-10, IL-13, TNFα, and IFN) during chronic inflammation. Thus, XENTICS , an antagonist of IL-4 receptor alpha, could modulate the formation of CYP450 enzymes. Therefore, upon initiation or discontinuation of XENTICS in patients who are receiving concomitant drugs which are CYP450 substrates, particularly those with a narrow therapeutic index, consider monitoring for effect (e.g., for warfarin) or drug concentration (e.g., for cyclosporine) and consider dosage modification of the CYP450 substrate.
Mechanism of Action
Dupilumab is a human monoclonal IgG4 antibody that inhibits interleukin-4 (IL-4) and interleukin-13 (IL-13) signaling by specifically binding to the IL-4Rα subunit shared by the IL-4 and IL-13 receptor
complexes. Dupilumab inhibits IL-4 signaling via the Type I receptor and both IL-4 and IL-13 signaling through the Type II receptor. Blocking IL-4Rα with dupilumab inhibits IL-4 and IL-13 cytokine-induced responses, including the release of proinflammatory cytokines, chemokines and IgE.
Pharmacodynamics
Consistent with receptor blockade, serum levels of IL-4 and IL-13 were increased following dupilumab treatment. The relationship between the pharmacodynamic activity and the mechanism(s) by which dupilumab exerts its clinical effects is unknown.

Absorption
Following an initial subcutaneous (SC) dose of 600 mg, dupilumab reached peak mean ±SD
concentrations (Cmax) of 70.1±24.1 mcg/mL by approximately 1 week post dose.

acné, alopecia, prurito o urticaria existe un mayor impacto a nivel psicológico con aparición de baja autoestima, temor al rechazo social, alteración del ánimo y en la calidad de vida e
incluso ideación suicida.
El acné es frecuente entre los adolescentes, con una prevalencia estimada de 80 a 90%. Un fármaco más usado en los casos más severos es la isotretinoína. Esta sustancia es un retinoide derivado de la vitamina A, con una vida media de 6 a 36 h, que posee varios metabolitos que se eliminan lentamente en el plasma y tienen efecto teratógeno. Por este motivo se recomienda el uso de anticoncepción por lo menos durante un mes después de suspender su administración.
Se han descrito otros efectos secundarios producidos por este fármaco como síntomas musculoesqueléticos, pseudo-tumor cerebral, disminución de la visión nocturna, opacidad
corneal y otras alteraciones mucocutáneas. Además puede producir una alteración de
parámetros analíticos como aumento de la VSG, elevación de glucemia y triglicéridos, alteración de la función hepática y disminución de HDL-colesterol. A pesar de todo lo
descrito anteriormente presenta altas tasas de curación total, por lo que es frecuente ver su
indicación por el dermatólogo y encontrarnos algún paciente medicado con esta sustancia en las consultas de atención primaria.
Menos frecuentes son los efectos adversos psiquiátricos, atribuidos a isotretinoína en
nuestra actividad asistencial, pero han sido registrados y descritos por diferentes expertos. En el sistema de informe de eventos adversos de la FDA, entre los años 1982 y 2002, se llegaron a presentar 3.104 casos de problemas psiquiátricos relacionados con este fármaco, principalmente depresión, crisis de pánico, psicosis, intentos de autolisis e incluso suicidio consumado.
Caso clínico
Presentamos el caso de una adolescente de 17 años, que fue derivada desde atención primaria a nuestra consulta de salud mental para valoración de síntomas depresivos.
Presenta antecedentes familiares de depresión en una tía paterna y como antecedentes
personales no se recogen alergias medicamentosas conocidas ni existencia de hábitos tóxicos conocidos. Aporta un electrocardiograma y una analítica general, incluyendo bioquímica y hormonas tiroideas, sin hallazgos significativos.
En los tres meses anteriores, inició tratamiento con isotretinoína a dosis de 20 mg/d prescrito por su dermatólogo por presentar acné nóduloquístico resistente a otros
medicamentos anteriores tópicos y sistémicos. La paciente refiere desde hace aproximadamente dos meses, síntomas de tristeza, sensación de anergia, apatía, dificultades de atención, pérdida de apetito y deseos de muerte sin ideación autolítica.
Además ha presentado una crisis de pánico e insomnio de conciliación, remitiendo ambos síntomas.
En una ocasión tuvo que asistir a consultas de urgencias por aparición brusca de sensación subjetiva de mareo, visión borrosa y sudoración asociada a sensación inminente de muerte que cedió tras administración de benzodiacepinas vía sublingual.
Finalmente fue remitida para control y seguimiento por atención primaria, informando a su médico de familia y al dermatólogo de una alta sospecha de crisis de pánico aislada y
episodio depresivo inducido por la toma de isotretinoína.
Translation - English Depression and panic attacks in acne treated with isotretinoin
There are many cases described in our daily clinical practice and in the medical literature concerning the reciprocal relation between psychiatric and dermatological diseases. Studies have identified among patients seeking treatment for skin disease a prevalence of
psychiatric pathology of 25% to 43%, mainly depressive, anxiety and somatoform disorders. Particularly in some pathologies such as acne, alopecia, pruritus or urticaria there is a greater psychological impact with the development of low self-esteem, fear of social rejection, changes in mood and quality of life, and even suicidal ideation.
Acne is common among adolescents, with an estimated prevalence of 80% to 90%. In the most severe cases, the drug most widely used is isotretinoin. This substance is a retinoid
derived from vitamin A, with a half-life of 6 to 36 hours, which has several metabolites that
are slowly eliminated in the plasma and have a teratogenic effect. For this reason, it is recommended to use birth control for at least one month after stopping its administration. Other side effects produced by this drug have been reported as musculoskeletal symptoms, pseudotumor cerebri, decreased night vision, corneal opacity and other mucocutaneous alterations. Furthermore, it can produce an alteration of analytical parameters such as an increase in ESR, elevated blood glucose and triglycerides, impaired liver function, and decrease in HDL-cholesterol. Despite everything previously described, it does have high cure rates; therefore it is common to see it being prescribed by dermatologists and to find a patient treated with this substance in primary healthcare centers.
Psychiatric adverse effects attributed to isotretinoin are less frequent in our welfare activities, but they have been recorded and reported by different experts. In the FDA's adverse event
reporting system, between 1982 and 2002, there were 3,104 cases of psychiatric problems
associated with this drug, primarily depression, panic attacks, psychosis, attempted suicide and even completed suicide.
Case report
We present the case of a 17-year-old girl, who was referred from primary care to our mental health clinic to be assessed for symptoms of depression.
She has a family history of depression in a paternal aunt, and in terms of personal history,
she has no known drug allergies or any existing toxic habits. She underwent an ECG and routine bloods tests, including biochemistry and thyroid hormones, without significant findings.
For the past three months, she has been treated with isotretinoin at a dose of 20 mg/d prescribed by her dermatologist for having nodulocystic acne resistant to other topical and
systemic medications. For approximately two months, the patient has reported symptoms of
sadness, lack of energy, apathy, concentration problems, loss of appetite, and death wishes without suicidal ideation.
Furthermore, she experienced a panic attack and conciliation insominia, with both symptoms
subsiding.
On one occasion, she had to visit the emergency room due to the sudden onset of lightheadedness, blurred vision and sweating associated with a feeling of imminent death
that relented following the sublingual administration of benzodiazepines.
She was finally referred for primary care monitoring and follow-up, with her family doctor and dermatologist being informed of a high suspicion of an isolated panic attack and depressive
episode induced by the intake of isotretinoin.

Artículo 63.
La concesión de nacionalidad por residencia se hará, previo expediente, por el Ministerio de Justicia. Las autoridades competentes para la tramitación y resolución de las solicitudes de adquisición de la nacionalidad por residencia, para la exclusiva finalidad de resolver la solicitud presentada por el interesado, recabarán de oficio de las Administraciones Públicas competentes cuantos informes sean necesarios para comprobar si los solicitantes reúnen los requisitos exigidos en el artículo 22 del Código Civil, sin que sea preciso el consentimiento de los interesados. Artículo 64.
A falta de disposición especial, es funcionario competente para recibir las declaraciones de conservación o modificación de nacionalidad o vecindad, el mismo que determina las reglas sobre opción de nacionalidad. Cuando dicho funcionario no sea el encargado del mismo Registro donde conste inscrito el nacimiento, levantará acta con las circunstancias exigidas para la inscripción y la remitirá al Registro competente para la práctica de la inscripción marginal correspondiente. Se considera fecha de la inscripción, a partir de la cual surten sus efectos tales declaraciones, la del acta que constará en dicho asiento.
Artículo 65.
La declaración a que se refiere el artículo 26 del Código Civil sólo puede hacerse dentro de un año a contar de la fecha en que la Ley del país de residencia atribuya la nacionalidad extranjera o desde la mayor edad o emancipación del declarante, si la Ley extranjera la hubiere atribuido antes. Una vez prestada la declaración de querer conservar la nacionalidad o vecindad civil, no es necesario reiterarla cualesquiera que sean el tiempo transcurrido o los cambios de residencia. Tampoco necesita prestar declaración de conservarla quien haya declarado su voluntad de adquirir la misma nacionalidad o vecindad.
Artículo 66.
Se inscribirán en el Registro Civil español las declaraciones y demás hechos que afecten a la condición jurídica de español o de nacional de país iberoamericano o de Filipinas de que, respectivamente, gocen conforme a los Convenios, los nacionales de estos países o los españoles. El encargado del Registro está obligado a comunicar estas inscripciones a la Dirección General de los Registros y del Notariado.
Artículo 67.
La pérdida de la nacionalidad se produce siempre de pleno derecho, pero debe ser objeto de inscripción. Caso de no promover ésta el propio interesado, el encargado del Registro, previa su citación, practicará el asiento que proceda.
Artículo 68.
Sin perjuicio de lo dispuesto en el Título 1, Libro 1 del Código Civil y en tanto no conste la extranjería de los padres, se presumen españoles los nacidos en territorio español de padres también nacidos en España. La misma presunción rige para la vecindad.

Article 64.
En l’absence de disposition spéciale, c’est un fonctionnaire compétent qui reçoit les déclarations de conservation ou de modification de la nationalité ou de la citoyenneté, le même qui détermine les règles en matière de choix de la nationalité. Si ce fonctionnaire n'est pas l'officier du même état civil où la naissance est inscrite, il rédige un procès-verbal contenant les conditions requises pour l'inscription et le transmet à l‘état civil compétent pour effectuer l'inscription concernée. L'on considère la date de l'inscription, à partir de laquelle les déclarations produisent leurs effets, celle du procès-verbal qui est inscrite dans l'entrée.
Article 65.
La déclaration visée à l’article 26 du code civil ne peut être faite que dans un délai d’un an à compter de la date à laquelle la loi du pays de résidence octroye la nationalité étrangère ou de l’âge de la majorité ou de l’émancipation du déclarant, si la loi étrangère l'avait octroyée auparavant. Dès que la déclaration de volonté de conserver la nationalité ou la citoyonneté régionale a été fournie, il n’est pas nécessaire de réaffirmer la déclaration quels que soient le délai écoulé ou les changements de résidence. L'individu qui a déclaré sa volonté d‘acquérir la même nationalité ou la même citoyenneté régionale ne doit pas non plus faire de déclaration de conservation.
Article 66.
Les déclarations et autres faits portant atteinte au statut juridique des Espagnols ou des ressortissants des pays ibéro-américains ou des Philippines dont ils bénéficient respectivement, conformément aux conventions, ainsi que les ressortissants de ces pays ou les Espagnols, seront inscrits à l‘était civil espagnol. L'officier d'état civil est tenu de communiquer ces inscriptions à la direction générale des registres et du notariat.
Article 67.
La déchéance de la nationalité se fait toujours à part entière, mais elle doit faire l'objet d'une inscription. Si l'intéressé lui-même ne promeut pas cela, l'officier d'état civil, au terme de sa convocation, effectue l'entrée concernée.
Article 68.
Sans préjudice du titre 1, livre 1 du code civil, et tant que le statut d‘immigration des parents n’est pas inscrit, les Espagnols nés sur le territoire espagnol des parents nés en Espagne sont présumés être des Espagnols. La même présomption s'applique à la citoyenneté régionale.

Source text - English H. pylori infection promotes carcinogenesis of GC
GC is one of many cancers associated with inflammation, which is induced by H. pylori infection [13, 14]. Furthermore, GC develops merely in persons with H. pylori infection, and is more prone to occur in patients with histologic findings of severe gastric atrophy, corpus- predominant gastritis, or intestinal metaplasia [15, 16]. On the other hand, H. pylori eradication may reduce the prevalence of precancerous gastric lesions and prevent the development of gastric cancer, even in those patients without precancerous lesions and in healthy asymptomatic infected Asian individuals [17-21]. In addition, metachronous gastric carcinoma can be prevented by H. pylori eradication in patients who received endoscopic resection of early GC [22]. These studies demonstrate that H. pylori infection is closely associated with gastric carcinogenesis.
H. pylori infection is associated with IL-1β expression
H. pylori infection induces IL-1β and suppresses acid secretion, while the expression of IL-1β decreases after H. pylori eradication, followed by an increase in gastric acidity [65, 66]. It is discovered that mucosal levels of IL-1β increases prior to the development of GC in an experimental mouse model with H. pylori infection [67]. In addition, gastric concentrations of IL-1β in children with H. pylori infection are significantly higher than those without H. pylori infection [68]. Furthermore, higher mucosal IL-1β levels are observed in H. pylori-infected GC patients with IL-1β-31TT compared to those with IL-1β-31CT and IL-1β-31CC [69]. On
the other hand, population with IL-1β-31 CT and TT genotypes in Asia and Latin America are more susceptible to infection by H. pylori, compared to those with IL-1β-31 CC [70]. Overall, these studies indicate that H. pylori infection has an interactive relationship with IL-1β, especially in patients with IL-1β-31TT.
Secretion of N- and O-linked Glycoproteins from 4T1 Murine Mammary Carcinoma
Cells
Breast cancer is one of the most common cancers that affect women globally and accounts for
~23% of all cancers diagnosed in women. Breast cancer is also one of the leading causes of death primarily due to late stage diagnoses and a lack of effective treatments. Therefore, discovering protein expression biomarkers is mandatory for early detection and thus, critical for successful therapy. Two-dimensional electrophoresis (2D-E) coupled with lectin-based analysis followed by mass spectrometry were applied to identify potential biomarkers in the secretions of a murine mammary carcinoma cell line. Comparisons of the protein profiles of the murine 4T1 mammary carcinoma cell line and a normal murine MM3MG mammary cell line indicated that cadherin-1 (CDH), collagenase 3 (MMP-13), Viral envelope protein G7e (VEP), Gag protein (GAG) and Hypothetical protein LOC433182 (LOC) were uniquely expressed by the 4T1 cells, and pigment epithelium-derived factor (PEDF) was exclusively secreted by the MM3MG cells. Further analysis by a lectin-based study revealed that aberrant O-glycosylated CDH, N-glycosylated MMP-13 and LOC were present in the 4T1 medium. These differentially expressed N- and O-linked glycoprotein candidates, which were
identified by combining lectin-based analysis with 2D-E, could serve as potential diagnostic and prognostic markers for breast cancer.

CG es uno de los muchos cánceres asociados con la inflamación inducida por la infección por H. pylori [13, 14]. y_1,…,y_N are instances of the output variable Y, which is called the response.
In linear regression, we assume that the response depends on the input variables in a linear fashion: y=f(X)+ε, where f(X)=β_0+β_1 X_1+⋯+β_p X_p.
Here, ε is called the error term and β_0,…,β_p are called parameters.
We don’t know the values of β_0,…,β_p. However, we can use the training data to approximate the values of β_0,…,β_p. What we’ll do is look at the amount by which the predicted value f(x_i) differs from the actual y_i for each of the pairs (x_1,y_1 ),…,(x_N,y_N ) from the training data. We want the residual sum of squares to be as small as possible. Essentially, this means that we want our predicted value f(x_i) to be as close to the actual value y_i as possible, for each of the pairs (x_i,y_i). Doing this will give us a linear function of the input variables that best fits the given training data. In the case of only one input variable, we get the best fit line. In the case of two input variables, we get the best fit plane. And so on, for higher dimensions.
THE LEAST SQUARES METHOD

Traditional structured reports though are often not tailored to the appropriate clinical situation, are generic, and can be overly constraining. Contextual reporting is an alternative method of structured reporting that is specifically related to the disease or examination indication. Herein, we create a library of 50 contextual structured reports for neuroradiologists and emphasize their clinical value over noncontextual structured reporting. ecent literature and several radiology organizations advocate the use of structured templates as a new standard for radiol- ogy reporting.1-5 Structured reporting has many advantages over traditional free-text, narrative reporting, including improved consis- tency, increased clarity, decreased grammar and speech recognition errors, increased ability to mine data for research, and greater finan- cial rewards.6-10 On the other hand, advantages of narrative or prose reporting include flexibility, personalization, and familiar- ity for radiologists. While structured reporting is not perfect, re- ferring providers have expressed greater satisfaction with the im- plementation of structured reporting.8 Despite this impetus, there are few data that objectively demonstrate the added clinical
value over narrative reporting.11

Not all structured reporting is created equally. Many of the published structured templates are technique- or examination- based.3 For example, in CT abdomen and pelvis imaging, structured reports are organized by a litany of organs.3 Similarly within neuro- imaging, a sample published MR imaging brain template contains 16 anatomic items within the findings section, and nearly all the fields are filled in with “normal.”12 While these reports may be versatile, they do not guide the radiologist during image interpretation but simply provide fields to report in a “structured” fashion. In addition, some radiologists are reluctant to use structured reporting because they think that the reports are too generic or simplistic and often not appropriately tailored to the clinical scenario.13
Contextual Structured Reporting
Contextual reporting is an alternative method of structured re- porting that is specifically related to the disease or examination indication. These disease-specific reports provide content focused on the clinical diagnosis or symptom, discuss appropriate differ- ential diagnoses, and highlight pertinent positives and negatives. Contextual reports are flexible; pick lists can be created within the templates to permit different options for the radiologist. Depend- ing on the reporting software, tiered concepts can also be man- aged to trigger other macros that can be inserted on the basis of specific keywords or selection of pick lists. Contextual reporting also does not preclude some free-text dictation within the appro- priate contextual fields. No template can perfectly fit every clinical scenario; thus, contextual templates should not be overly restric- tive. This feature is another negative that some radiologists have found with traditional structured reporting.14

Contextual structured reporting has only been applied to a handful of diseases, including pancreatic cancer, pulmonary em- bolism, pediatric Crohn disease, prostate cancer, rectal cancer, infertility, and interstitial lung disease.11,15-20 To the authors’ knowledge, contextual templates for multiple sclerosis, thyroid imaging, and head trauma are the only contextual structured re- ports developed within neuroimaging.21-23 The purpose of this article was to create a library of 50 contextual structured reports for neuroradiologists and emphasize their clinical value over non- contextual structured reporting through the use of selective im- aging examples.
Template Creation Process
The contextual templates were created in a systematic process that spanned >1 year, which is similar to processes in other institu- tions.24,25 Our institution (Kaiser Permanente, Santa Clara, Cal- ifornia) is part of a 21-hospital network comprising 300 radiolo- gists, 50 of whom are neuroradiologists. Led by a committee chairperson, a committee search was initiated to identify radiol- ogists interested and knowledgeable about structured reporting and informatics. The subspecialty regional leaders of neuroradi- ology within brain, neurovascular, head/neck, pediatric neurora- diology, and spine imaging were recruited. After the committee was created, common examination indications and diseases within neuroradiology were culled from examination data, and templates were drafted. For each template created, the com- mittee members provided revisions and ensured that the tem- plates contained content contextual to the disease process or examination indication. Following committee voting and ap- proval, each of the templates was sent to a subspecialty clini- cian who was an expert in the specific topic to provide addi- tional suggestions. Before template publication within our radiology dictation software (Radiator; Napa, California), the informatics leader of the committee coded the template, en- suring the templates were practical to use and creating pick list options when appropriate.
The templates were mapped to the procedural codes of our institution within the radiology information system, which are closely modeled after the current procedural terminology codes, with the exception of more granularity to permit more complex associations between the procedures and structured templates. This mapping permits the template to automatically load in the reporting software when the examination is opened for interpre- tation. The templates were then made available to all radiologists to use in clinical practice, and a hyperlink was inserted within the dictation software for users to provide suggestions for change, which were reviewed and revised when appropriate. To orient the radiologists with the contextual templates at the onset of deploy- ment, the template committee gave a lecture on the types of tem- plates developed, the advantages of contextual reporting, and ref- erence to available imaging guides for certain templates.

Processus de création de modèle
Les modèles contextuels ont été créés selon un processus systématique s'étalant sur un an, ce qui est similaire aux processus d'autres institutions. 24,25 Notre établissement (Kaiser Permanente, Santa Clara, Cal-ifornia) fait partie d'un réseau de 21 hôpitaux comprenant 300 radiologistes, dont 50 neuroradiologues. Sous la direction d'un président de comité, une recherche de comité a été lancée pour identifier les radiologistes intéressés et compétents en matière de rapport et d'informatique structurés. Les leaders régionaux de la neuroradiologie sur la surspécialité en imagerie cérébrale, neurovasculaire, tête / cou, neurora-diologie pédiatrique et de la colonne vertébrale ont été recrutés. Après la création du comité, des indications d’examen communes et des maladies en neuroradiologie ont été extraites des données d’examen et des modèles ont été élaborés. Pour chaque modèle créé, les membres du comité ont fourni des révisions et ont veillé à ce que les modèles contiennent un contenu contextuel par rapport au processus de la maladie ou à l'indication de l'examen. Suite au vote et à l'approbation du comité, chacun des modèles a été envoyé à un sous-spécialiste, expert en la matière, pour lui faire part de suggestions supplémentaires. Avant la publication des modèles dans notre logiciel de dictée de radiologie (Radiator; Napa, Californie), le responsable informatique du comité avait codé le modèle, ce qui rendait l'utilisation des modèles pratique et la création d'options de liste de choix, le cas échéant.
Les modèles ont été mis en correspondance avec les codes de procédure de notre institution dans le système d’information radiologique, qui sont très proches des codes de terminologie de procédure actuels, à l’exception de la plus grande précision permettant des associations plus complexes entre les procédures et les modèles structurés. Ce mappage permet au modèle de se charger automatiquement dans le logiciel de création de rapports lorsque l'examen est ouvert pour interprétation. “We should not argue with people in need; we should help them.”
Since January 1st, 2013 our organization is an independent foundation of the Order of Malta Switzerland, registered as a non-profit entity. The Foundation has a flat organization and the work is until today done entirely on a voluntary basis. The core activity of the Foundation is to provide countries in need with well-kept second hand material from Swiss hospitals and schools. In the mean time we have developed a large network consisting of many hospitals, community homes, schools and companies active in medical equipment and hospital furniture.
Goods are provided by the university hospitals of Zurich, Lausanne, Geneva, Bern as well as the hospital network of Fribourg, all community homes and local hospitals of the City of Zurich, hospitals in Wallis, the rehabilitation clinic in Basel, the cantonal hospitals of Lucerne and Glarus, the clinic Sonnenhof and the hospital Engeried in Bern, the hospital STS Thun, the clinics of the Groupe Genolier, the Hirslanden clinics, the hospital in Biel, the companies Cosanum Schlieren, Kuoni in Zurich, the hospital beds manufactures Bigla, Embru and Hillrom, and many others.
All in all, we have received goods from 55 hospitals and 325 community homes over the last few years, in support of our projects all over the world.
Thanks to our reliable and timely way of working, the Foundation has become a reliable partner for our material donors, allowing them to provide needy institutions with relief material at no cost. During 2018 we have continued to strengthen the cooperation with the Order of Malta in different countries, in particular with the Order in Poland, Slovakia, Mauritania, Chile, Bulgaria and Hungary. The cooperation with the Embassies of the Order has improved markedly.
The countries involved are Slovakia, Poland, Portugal, Nepal, Ghana, Hungary, Albania, Bosnia, Serbia, Bulgaria, Ukraine, Chile, India, Ghana and others. We have as well given support to some institutions in Switzerland. During 2018 we had the opportunity to again cooperate actively with the Swiss Red Cross for projects in Bosnia. This cooperation will continue in 2019, creating a positive win-win situation for both organizations. Aid is provided to all countries, without consideration of nationality or religion, in the spirit of the Order “Tuitio fidei et obsequium Pauperum”.
Activities of the Foundation in 2018
Operations:
During 2018 we have sent 62 semi-trailerss and maritime containers for a total volume of 9’000 cubic meters and a total weight of 2‘000 tons. Compared to the previous year we were able to halve the number of semi-trailers with a new loading method: we load twice as many beds in one truck by straitening the beds, instead of staking them one on top of the other. The commercial value is estimated at 20 million Swiss Francs. We have sent 2’710 beds and 400 school desks in addition to all the other goods. Compared to 2017 we have maintained the same volume. Our budget amounted to 170’000.- CHF.
The volunteers of the Foundation have worked for a total of 4’500 hours in 2018. Payed staff would have costed about 350’000.- CHF for the same amount of work.
A further increase of our aid transports is not possible with our current structure and resources.
We had very generous assistance of the staff of Ernst &Young, Lions Club, local confirmands, the Basket Fribourg Olympics, many sections of the Maltese Hospital Service (SHOMS).
As Operational Director I had the opportunity to give a number of conferences this year. Our cooperation with the Embassies of Slovakia, Bulgaria and Chile in Switzerland is very efficient. We also have an excellent cooperation with the Order of Saint John (Johanniter) with which we sent 5 trailers.

In order to ensure the future of the foundation after the retirement of the current volunteers (aged 75 on average) and myself, we took a number of initiatives:
1) We identified and recruited a new Operational Director. Andreas Jaross, former ICRC delegate, active member of the SHOMS and candidate to join the Order in June 2019 will start as of March 2019.
His presence and professional experiences are clear signs to donors that we want the show to go on.
2) Andrea Stopper, former Executive Vice-President at Fresenius, joined us as Trésorier.
3) Alessandro Marangoni, who was already a member of our Board, took of the presidency of the foundation.
4) The rest of our board, except for the resignation of the Swiss Hospitalier Paolo Kramer for health reasons, remains unchanged and fully engaged.
However, we get to the limits of our skills and available time.
5) Among 7 different candidates, we selected a fundraiser who is structuring our fundraising efforts in the German part of Switzerland to start with.
6) In order to engage large donors, we are refreshing our Statutes. The first version was not accepted by the Swiss authorities, we are in direct contact with them to satisfy their expectations and hope to close this topic soon as it will open doors with certain large donors.
7) We moved our website to a professionally run server simplified it, and translated it to English.
8) We started communicating actively on Facebook.
9) SalesForces, the world leading CRM solution provider, selected our foundation as pilot partner for its non-profit solution is D-A-CH. This will not only help keeping track of our donors as their number grows, but could also give us visibility through their marketing communication.
10) Finally, we gave up a 20 years old lease for a storing facility in Geneva which will save us CHF 4’000.- in 2019 already (thereafter CHF 5’000.-).
Finances:
In 2019, we can count on the Global Fund for Forgotten People to cover the salary of Andreas Jaross. However, we are still missing CHF 40’000.- to close our budget as our traditional donors from the Order (FONDASHOMS and FONDAHOM) cannot help us anymore.
As of 2020, when all my volunteers will have retired, we will need CHF 450’000.- per year to maintain the same operational level.
Our fundraising efforts have started in the autumn 2018 and we are currently following up with the donors we have direct contacts to. We are exploring all opportunities, but reach the limits of what our Board and myself can do. Decisive support is required for the Foundation to continue its work for our Lords the Sick and the Order.

Depuis le 1er janvier 2013, notre organisation est une fondation indépendante de l'Ordre de Malte en Suisse, enregistrée en tant qu'entité à but non lucratif. La Fondation a une organisation horizontale et le travail est jusqu’à présent accompli entièrement à titre bénévole. L'activité principale de la Fondation est de fournir aux pays dans le besoin du matériel de seconde main bien entretenu provenant d'hôpitaux et d'écoles suisses. Entre temps, nous avons développé un vaste réseau comprenant de nombreux hôpitaux, maisons communautaires, écoles et entreprises actives dans le secteur des équipements médicaux et du mobilier hospitalier.
Les biens sont fournis par les hôpitaux universitaires de Zurich, Lausanne, Genève, Berne ainsi que par le réseau hospitalier de Fribourg, tous les foyers et hôpitaux locaux de la ville de Zurich, les hôpitaux de Wallis, la clinique de rééducation de Bâle, les hôpitaux cantonaux de Lucerne et Glaris, la clinique Sonnenhof et l'hôpital Engeried à Berne, l'hôpital STS Thun, les cliniques du Groupe Genolier, les cliniques Hirslanden, l'hôpital à Biel, les sociétés Cosanum Schlieren, Kuoni à Zurich, les fabricants de lits d'hôpital Bigla, Embru et Hillrom, et beaucoup d'autres.
Au total, nous avons reçu des marchandises de 55 hôpitaux et de 325 foyers communautaires au cours des dernières années, pour appuyer nos projets partout dans le monde.
Grâce à notre manière de travailler fiable et opportune, la Fondation est devenue un partenaire fiable pour nos donateurs en matériel, leur permettant de fournir gratuitement du matériel de secours aux institutions dans le besoin. En 2018, nous avons continué de renforcer la coopération avec l'Ordre de Malte dans différents pays, en particulier avec l'Ordre en Pologne, en Slovaquie, en Mauritanie, au Chili, en Bulgarie et en Hongrie. La coopération avec les ambassades de l'Ordre s'est nettement améliorée.
Les pays concernés sont la Slovaquie, la Pologne, le Portugal, le Népal, le Ghana, la Hongrie, l’Albanie, la Bosnie, la Serbie, la Bulgarie, l’Ukraine, le Chili, l’Inde, le Ghana et d’autres. Nous avons également apporté un soutien à certaines institutions en Suisse. En 2018, nous avons eu l'occasion de coopérer à nouveau activement avec la Croix-Rouge suisse pour des projets en Bosnie. Cette coopération se poursuivra en 2019, créant une situation positive gagnant-gagnant pour les deux organisations. L’aide est fournie à tous les pays, sans distinction de nationalité ou de religion, dans l’esprit de l’ordre «Tuitio fidei et obsequium Pauperum».
Activités de la Fondation en 2018
Opérations :
En 2018, nous avons envoyé 62 semi-remorques et conteneurs maritimes pour un volume total de 9 000 mètres cubes et un poids total de 2 000 tonnes. Par rapport à l'année précédente, nous avons pu réduire de moitié le nombre de semi-remorques avec une nouvelle méthode de chargement: nous chargeons deux fois plus de lits dans un camion en les resserrant au lieu de les superposer. La valeur commerciale est estimée à 20 millions de francs suisses. Nous avons envoyé 2 710 lits et 400 pupitres d'école, en plus de tous les autres biens. Par rapport à 2017, nous avons maintenu le même volume. Notre budget s'est élevé à 170'000.CHF
Les bénévoles de la Fondation ont travaillé pendant 4 500 heures en 2018. Le personnel rémunéré aurait coûté environ 350 000 personnes.- CHF pour la même quantité de travail.
Une augmentation supplémentaire de nos transports d’aide n’est pas possible avec notre structure et nos ressources actuelles.
Nous avons bénéficié de l'assistance très généreuse du personnel d'Ernst & Young, du Lions Club, des confirmands locaux, des Jeux olympiques Basket Fribourg et de nombreuses sections du Service hospitalier maltais (SHOMS).
En tant que directeur des opérations, j'ai eu l'occasion de donner un certain nombre de conférences cette année. Notre coopération avec les ambassades de Slovaquie, de Bulgarie et du Chili en Suisse est très efficace. Nous avons également une excellente coopération avec l'Ordre de Saint John (Johanniter) avec lequel nous avons envoyé 5 remorques.
Organisation :
Afin de garantir l'avenir de la fondation après le départ à la retraite des bénévoles actuels (âgés en moyenne de 75 ans) et de moi-même, nous avons pris un certain nombre d'initiatives:
1) Nous avons identifié et recruté un nouveau directeur opérationnel. Andreas Jaross, ancien délégué du CICR, membre actif du SHOMS et candidat à l'adhésion à l'Ordre en juin 2019, commencera à compter de mars 2019.
Sa présence et ses expériences professionnelles sont des signes clairs pour les donateurs que nous souhaitons que le spectacle continue.
2) Andrea Stopper, ancienne vice-présidente exécutive chez Fresenius, nous a rejoint en tant que Trésorier.
3) Alessandro Marangoni, qui était déjà membre de notre conseil, a assumé la présidence de la fondation.
4) Le reste de notre conseil, à l'exception de la démission de l'hospitalier suisse Paolo Kramer pour des raisons de santé, reste inchangé et pleinement engagé.
Cependant, nous arrivons à la limite de nos compétences et de notre temps disponible.
5) Parmi 7 candidats différents, nous avons sélectionné une collecte de fonds qui commence par structurer nos efforts de collecte de fonds en Suisse alémanique.
6) Afin de mobiliser de grands donateurs, nous actualisons nos statuts. La première version n’a pas été acceptée par les autorités suisses. Nous sommes en contact direct avec elles pour répondre à leurs attentes et espérons clore ce sujet prochainement car il ouvrira des portes à certains grands donateurs.
7) Nous avons transféré notre site Web sur un serveur géré par des professionnels, nous l'avons simplifié et traduit en anglais.
8) Nous avons commencé à communiquer activement sur Facebook.
9) SalesForces, le principal fournisseur mondial de solutions de gestion de la relation client, a choisi notre fondation comme partenaire pilote pour sa solution à but non lucratif appelée D-A-CH. Cela aidera non seulement à suivre nos donateurs au fur et à mesure que leur nombre augmente, mais pourrait également nous donner une visibilité grâce à leur communication marketing.
10) Enfin, nous avons renoncé à un bail de 20 ans pour une installation de stockage à Genève, ce qui nous fera économiser 4 000 CHF.- déjà en 2019 (ensuite CHF 5'000.-).
Finances:
