This is a new type of marriage registration in the UK and am unsure how to render it in French.
Below is an extract from a website in a Town Hall in the UK:
Civil Partnership, Marriages and Deaths (Registration etc.) Act 2019 will modernise how marriages are registered for the first time since 1837, through the introduction of a marriage schedule system and registration in an electronic register.
The electronic register will be a more secure and more efficient system for keeping marriage records. It will also allow for the names of parents of the couple (mother/father/parent) to be included in the marriage entry and on marriage certificates.
There will be no transitional period. In other words, marriages in England and Wales taking place on or after 4 May 2021 must do so under the new schedule system, even where civil or ecclesiastical preliminaries have already taken place.
A ‘marriage document’ (CofE / CinW) or ‘marriage schedule’ (all other) will be used by the specified person to record the details of the marriage for the register. The document or schedule must be returned to the local Register Office within 21 days to enable the registration to take place. Only then will marriage certificates be issued.
There will no longer be a marriage register or duplicate marriage register or certificates at ceremonies. All marriages taking place in any church or registered building will now be registered after the ceremony by the Local Registration Service (LRS).
The regulations which will make the required amendments to the Marriage Act 1949 were laid in Parliament on 22 February 2021, and the associated regulations will come into force on 4 May 2021.
If you are a specified person responsible for the solemnisation and / or registration of marriages in Church of England / Church in Wales, an Authorised Person, a Secretary (of marriages) of Synagogues or Marriage Officer for the Society of Friends, you will need to be aware of the following information.

Selon les infos fournies, ce schedule est synonyme de document en Angleterre et au pays de Galles, ce qui aide pour la traduction.
On nous dit qu'il sert à record the details of the marriage for the register et qu'il must be returned to the local Register Office... to enable the registration to take place. Only then will marriage certificates be issued.
Donc un document d'information et non pas un certificat.
Puisqu'il s'agit de noter des infos (dont on suppose qu'elles sont demandées), pourquoi pas « questionnaire » ? C'est ce que j'avais compris grâce aux guillemets que vous avez placés. D'où ma suggestion de traduction. Puisque c'est celui dont vous recherchez la traduction, ce terme doit apparaître seul dans la zone de la question. Le reste pouvait aller dans la zone d'explication, puisque comme vous le dites vous-même, il s'agit d'une phrase/d'un contexte dans lequel vous l'avez mis, comme vous dites, et qui ne vous pose donc pas de problème particulier.
L'intérêt de respecter la zone de la question proprement dite tient à la constitution du glossaire : schedule tel qu'il est employé ici mérite d'y entrer, mais marriage solemnized ou Civic Centre, s'ils n'y sont pas déjà, ne sont pas particulièrement difficiles à traduire.
Speaking of which, je me demande si la phrase dans laquelle vous l'avez mis n'est pas trompeuse. Je n'ai trouvé aucun exemple d'un schedule qui solemnize un mariage. En fait, solemnize a marriage, c'est le faire célébrer (https://dictionary.cambridge.org/fr/dictionnaire/anglais/sol... OU https://www.collinsdictionary.com/dictionary/english/solemni... par une personne autorisée. Or, ce schedule ne célèbre pas le mariage, mais il est destiné à recueillir des renseignements sur le futur couple pour que le mariage puisse être enregistré/officialisé/whatever par le Register Office.

Mise au point :
Le message qu'Emmanuella cite dans la discussion n'est pas celui qui a été supprimé par le personnel du site pour violation des règles 3.5 et 3.7.
Je ne sais pas pourquoi celui dont elle parle a disparu. Quoi qu'il en soit, mon italien n'est pas suffisant pour juger de la valeur de la question et des réponses apportées. Mais rien n'exclut que l'auteur de la question n'ait lui aussi tenté une mise en contexte personnelle malheureuse. C'est déjà arrivé ici.
Quoi qu'il en soit, j'attends toujours qu'on me montre un document officiel britannique qui cite verbatim l'expression dont Nicole demande la traduction. Je n'ai trouvé « marriage solemnized by schedule » nulle part et certainement pas dans le texte de l'administration britannique cité par Emmanuella.
Je suis naturellement prêt à revenir sur ma position quand je pourrai consulter un tel document.
